The left-back has impressed for Augsburg in the Bundesliga this season and could replace Filipe Luis, who seems keen on returning to Atletico Madrid.
Luis joined the Blues for £16 million from the Rojiblancos last year, but failed to earn the left-back spot at Stamford Bridge - with Mourinho preferring to play Cesar Azpilicueta out of position ahead of the Brazilian.
Baba Rahman could follow the footsteps of former Ghanaian Chelsea star, Michael Essien. The 20-year-old made 31 appearances for Augsburg this season, assisting five goals.
He made his Black Stars debut in September 2014 and was part of the Ghana squad which finished runners-up at the 2015 Africa Cup of Nations.
