The 35-year-old will join Qatari side Al Sadd this summer, but bowed out in style. Saturday's Champions League trophy was the Spaniard's 25th club trophy with Barcelona in 18 years.
Following the Copa del Rey triumph, Xavi had already surpassed Paco Gento's record for most trophies won by a Spanish player.
He also set up another record with his final Champions League appearance in Berlin, becoming the most capped player in the history of the competition.
He has now played 151 games, while Real Madrid duo Iker Casillas (150) and Raul (142) follow in second and third.
Xavi has now won eight leagues, three Spanish cups, six Spanish Super Cups, four Champions Leagues, two European Super Cups and two FIFA Club World Cups.
Added to Xavi's 25 club trophies, he has won two European Championships and the World Cup with Spain.

The team is unbelievable and so are the fans. I'm also feeling a bit nostalgic, knowing that I won't ever play for this team is tough to take in but the end has been incredible."
