Portugal will have to go through the play-offs after suffering a 2-1 defeat to Serbia, who qualified for the World Cup on Sunday night.
Renato Sanches opened the scoring for the home side after just two minutes, but they were pegged back in the 33rd minute through Dusan Tadic's equaliser.
With time running out, Aleksander Mitrovic proved to be the hero by netting the winner for Serbia in second-half stoppage time to send his side to the World Cup.
Spain 1-0 Sweden
Spain secured their ticket to the showpiece next year after defeating Sweden 1-0 in their final Group B clash.
While a point would have been enough, Alvaro Morata got the winning goal for La Roja in the 86th minute to seal top spot in the group.
Sweden still have a chance at qualifying, but will now fall into the play-offs.
Armenia 1-4 Germany
Germany finished their qualifying campaign with a 4-1 victory over Armenia.
After becoming one of the first sides to qualify in the last FIFA break, the Germans did not have any pressure to deal with.
Kai Havertz and Illkay Gundogan put the visitors two goals up in the first half.
Gundogan and Jonas Hoffman completed the rout for Germany, while Henrikh Mkhitaryan bagged a consolation for Armenia.
Croatia 1-0 Russia
Croatia qualified for the World Cup in Qatar with a 1-0 win over Russia on Sunday evening.
The Russians were minutes away from sealing top spot on Sunday until an own goal Fyodor Kudryashov in the 81st minute handed Croatia three points and a spot at the World Cup.
Russia, meanwhile, finished second in the group and will compete in the play-offs.
