Maurizio Sarri's first season in charge of the Blues was a bumpy one, although they have secured Champions League football for next season and can still finish the campaign with a trophy.
One player who has been key to the side's success is Belgian star Eden Hazard, who has registered 19 goals and 16 assists across all competitions.
The Belgian has been rewarded for his performances after being named Player of the Year and Players' Player of the Year.

Hazard also scooped up the Goal of the Season award for his solo strike against Liverpool, while higly-rated Callum Hudson-Odoi was named Young Player of the Year.
