Bayern manager Thomas Tuchel made only one change from their thumping win over Augsburg, as Thomas Muller was recalled and joined Kingsley Coman and Leroy Sane as the front three behind lone-striker Harry Kane.
The Bavarians started at a good tempo and created decent chances in the early stages but Gladbach came the closest to breaking the deadlock on 26 minutes after Mavin Friedrich's header came off the crossbar from a cross by Alassane Plea.
Gerardo Seoane's charges eventually went ahead on the half-hour mark after Ko Itakura pounced on a flick on by Maximilian Wober with a header at the back post from a corner delivery by Plea.
Gladbach nearly capitalized on their momentum four minutes later, as Maximilian Weigl had a crack at goal on the edge of the box that produced great reflexes from goalkeeper Sven Ulreich.
Bayern were fired up and came very close to restoring parity just before the interval after Sane showed a great burst of pace to go through on goal after a pass by Muller and beat shot-stopper Moritz Nicolas with a close-range shot but his effort crashed against the post.
The Bavarians were all over Gladbach from the break and nearly equalised ten minutes in after Leon Goretzka's goal-bound header was cleared off the line by Itakura.
The visitors' persistence finally paid off on the hour-mark, as Sane levelled matters with a close-range shot after a ball over the top by Joshua Kimmich.
Chasing the winner with 20 minutes to go, Tuchel made a double attacking change, as Eric Maxim Choupo-Moting and Serge Gnabry were introduced in places of Muller and Coman.
Gnabry had a big chance to give Bayern the lead just after coming on after meeting a precise cross by Konrad Laimer at the back post but he headed his effort straight at shot-stopper Moritz Nicolas.
The game was on the edge in the dying minutes, as Gladbach also threatened on counter-attacks but Mathys Tel finally snatched the winner for the Bavarians three minutes before time with a downward header from a cross by Kimmich, just six minutes after coming on.
Choupo-Moting almost killed off the game in stoppage time with a header towards the bottom corner that was brilliantly kept out by Nicolas but the Bavarians had done enough to extend their winning start to three matches.
