Melinda Kgadieta was restored to the Banyana line-up after starting on the bench against Tunisia, with Mapula Kgoale dropping to the bench.
The Zambians had a big chance to break the deadlock after eight minutes when Ochumba Lubandji was left un-marked at the near post, but she failed hit the target.
Banyana, meanwhile, struggled to make any forays into the Zambian final third in open play and as a result, struggled to force any serious saves out of goalkeeper Catherine Musonda.
At the other end, Andila Dlamini was the busier of the two goalkeepers in the opening 45 minutes although she did manage to hold her own whenever the Zambians unleashed efforts at goal.
Desiree Ellis opted for a double change at the start of the second half, with Kgoale and Hildah Magaia replacing Thalea Smidt and Kgadiete.
Banyana thought they had been awarded a penalty just before the hour mark for Lushomo Mweemba's foul on Noxolo Cesane but after a check from the Video Assistant Referee (VAR), a foul was given instead as the challenge took place outside of the box.
With extra-time looming, Bafana nearly found the breakthrough as Magaia exchanged pass with Jermaine Seoposenwe before seeing her close-range strike tipped away by Musonda.
However, Banyana were given a lifeline in the second minute of time added on after being awarded a penalty following a VAR review. Linda Motlhalo stepped up and calmly converted the spot-kick, which sealed the win and Banyana's spot in the WAFCON final.
