The Blues recently sold Brazilian defender Felipe Luis back to Atletico Madrid, and have since been in the market for a replacement.
Rahman's agent, Sascha Empacher, has confirmed that his client has agreed personal terms with Mourinho's side, with just the clubs left to reach an agreement on a fee.
"It is only the clubs that have to find an agreement," he told Sport Bild.
"We on our side are agreed."
Rahman's German club have resigned themselves to losing the 21-year-old, and have acted by signing Philipp Max from Karlsruhe as his replacement.
