Ahly, who targeted back-to-back Super Cup triumphs under Pitso Mosimane, preferred South African Percy Tau but a change before kickoff saw Yasser Ibrahim replace Amr El-Sulya.
Following a lively start, the Cairo Red Devils were behind 13 minutes in after a galloping run from Ahmed Hafidi led to a cross which Hamid Ahadad tucked home after Ahly failed to clear their lines.
Ahly looked to draw level after two minutes from a half-volley on the edge of the area by Ayman Ashraf, but his attempt was beaten away by their opposite number.
The Cairo Red Devils continued to ask questions of their North African counterparts just inside the latter periods of the half with Tau on the end of a move from close-range, but he was courted.
Raja, who were buoyed by the fast start, could've gone on to double their advantage in the closing exchanges but instead applied a low defensive block to prevent Ahly a look in.
As the African champions withdraw Ashraf for Mohamed Sherif ahead of the second half, Hamdy Fathy nodded a looping ball from a corner-kick at the near post over target.
The Cairo Red Devils applied more pressure with a quarter of the contest approaching and came close to draw level from a half-volley outside the area by Mohamed Magdy Afsha.
Chance after chance came for Ahly well inside the closing stages and calls for a penalty waved away from a move by Tau.
Ahly continued to camp in Raja's half and levelled in the last minute of regulation time when Tau played a cutback to Taher Mohamed to slot in from close-range following a loose ball.
The match went to a penalty shoot-out to determine the winner following the stalemate, and Ahly emerged victors after Ahmed Tawfik netted the winning strike while Abdelilah Madkour missed at the other end.
