Sundowns made only one change for the final as Thapelo Morena took the place of attacker Sibusiso Vilakazi in the line-up while the Citizens were unchanged.
The game got off to a lively start as Lyle Lakay and Rivaldo Coetzee forced saves out of shot-stopper Hugo Marques while Mduduzi Mdantsane squandered a good chance at the other end.
The deadlock was broken on 24 minutes as Kutumela won possession by pressing high on the right wing and showed greta composure to pick out Morena, who fired into an empty net.
Eric Tinkler's charges were unfortunate not to equalise two minutes later after Craig Martin latched onto a through ball inside the box, only to see his effort saved by shot-stopper Denis Onyango from point-blank range.
Thamsanqa Mkhize had a chance to level matters for the Citizens just before the half-time break after going clear on goal down the right side of the box but he ballooned his strike over the bar asSundowns took their 1-0 lead to the half-time interval.
City looked to get themselves back on level terms after the break and they went clsoe four minutes in when Fasika scuffed a chance at the far post from a well-taken free-kick.
Chances were flying in at both ends as Zwane rattled the frame of the goal on the hour mark before Morena squandered a glorious one-on-one chance six minutes later after firing straight at Marques.
Tinkler re-shuffled his pack for the final 20 minutes as Terrence Mashego and Mpho Makola replaced Martin and Mdantsane as they went in search of the equaliser.
City got their reward soon after as Ralani turned provider to play in Fagrie Laky who took once touch before burying the ball into the far corner to restore parity.
Lakay's goal would ultimately set up extra-time after neither City or Sundowns could find a winning goal in the final 15 minutes of regulation time.
City, however, were forced to finish the game with 10 men as Idumba Fasika was given his marching orders after picking up a second yellow card.
The Citizens held on in the closing stages of extra-time to take the game to penalties, where Sundowns managed to triumph after a dreadful shoot-out that went to sudden death.
