Kai Havertz has faced a lot criticism during his debut season with the Blues, but he was the hero on the night after netting the only goal in the first half.
It was also Havertz's first-ever goal in the Champions League, having failed to score in the previous rounds or for Bayer Leverkusen.
There was also redemption for manager Thomas Tuchel who picks up his first Champions League title after finishing as runners-up in last season's final against Bayern Munich with Paris Saint-Germain.
Chelsea will now battle Europa League winners Villarreal in next season's UEFA Super Cup tie.
