Sarri made only one change from the weekend with Juan Cuadrado slotting in at right full-back in place of Danilo, while Napoli were missing the suspended goalkeeper David Ospina, with Alex Meret taking over between the sticks.
Juventus almost had a dream start in the fifth minute after Paulo Dybala won possession high up the pitch and played a through ball towards Cristiano Ronaldo, whose low shot was saved by Meret.
Despite a laboured opening 15 minutes, the Blues nearly broke the deadlock close to the half-hour mark after Lorenzo Insigne bent a free-kick from a dangerous position that came off the post.
The game came to life in the closing minutes of the first stanza as Ronaldo was denied again by Meret while Juventus shot-stopper Gianluigi Buffon pulled off a fine save with his legs to keep Napoli's Diego Demme at bay, as the sides went nto the break goalless.
The opening exchanges of the second stanza were tentative, with neither side willing to take the game by the scruff of the neck.
However, there was a spark in the match after the hour-mark following the introduction of fresh legs, as Danilo came on for Douglas Costa for the Bianconeri while Arkadiusz Milik replaced Dries Mertens for Napoli.
Milik nearly broke the deadlock for Gennaro Gattuso's charges with 18 minutes remaining on the clock after receiving a cut-back from Matteo Politano but he struck his shot over the bar.
The sides upped their efforts in the closing period, with the prospect of the penalty shoot-outs dawning upon them but they were both let down by their finishing in the last third as the match ended without goals in regulation time.
Dybala and Danilo both failed to convert their penalty kicks early on, giving Napoli the advantage as Gattuso's troops kept their nerve to win 4-2 on penalties, with Milik scoring the winning spot-kick.
