Selecao coach Roberto Martinez recalled Vitinha in midfield as Ruben Neves dropped out, while Spain also made a change of their own by brining in Fabian Ruiz for Mikel Merino.
La Roja should have opened the scoring on 15 minutes when Nico Williams broke free down the left before pulling the ball back for Pedri, who sent his first-time strike inches wide of the upright.
Spain took the lead on 22 minutes when Martín Zubimendi pounced on Joao Neves' clearance in the box and fired into the back of the net with a first-time finish.
However, Portugal hit back five minutes later as full-back Nuno Mendes raced clear into the box after a pass from Pedro Neto before rifling the ball into the far corner for his first international goal.
Both sides continued pushing for the go-ahead goal, which eventually fell to Spain on the stroke of halftime when Pedri turned provider to play in Mikel Oyarzabal, who poked the ball beyond the on-rushing Diogo Costa.
The lively tempo carried over to the second period as Portugal were level for a second time when Mendes turned provider to pick out Ronaldo, who volleyed home his eighth goal in the tournament on 62 minutes.
Spain responded with a double change on 74 minutes as Isco and Merino came on for Pedri and Ruiz, while Rafael Leoa and Renato Veiga were also introduced for Portugal.
The game did go on to extra-time but Portugal were forced into a change before the end of regulation time as Ronaldo was unable to continue and was replaced by Goncalo Ramos.
The two sides could not find a breakthrough in the 30-minute period, forcing the game to a penalty shoot-out where Portugal held their nerve to convert all five spot-kicks while Costa saved Alvaro Morata's spot-kick to hand the team their second Nations League title.
