Milan closed in on an equaliser, but a stunning save from Samir Handanovic blocked Lucas Paqueta's powerful shot on eight minutes.
Inter should've made it two on the half-hour mark, but Vecino failed to add a second to his name with a dismal finish from inside the box.
In the final chance of the half, Milan Skriniar sliced a big opportunity from a corner to put Inter two ahead at halftime on 41 minutes.
Three minutes into the second half, Roberto Gagliardini's curling shot produced a one-handed save from Gianluigi Donnarumma on 48 minutes.
Inter increased their advantage in the 51st minute as Dutch international Stefan De Vrij nodded into the bottom corner from Matteo Politano's cross.
Milan revived their stall as Tiemoue Bakayoko's powerful header beat Handanovic from a Calhanoglu's free-kick on 58 minutes.
The Nerazzurri were awarded a penalty after Politano was fouled in the area. Martinez restored their two-goal advantage on 67 minutes.
Milan set up a nervy end to match, as Mateo Musacchio lashed home a loose ball from inside the box on 71 minutes.
The Rossoneri had Andrea Conti dismissed for a second for a reckless challenge on Martinez but with help from the Video Assistant Refereeing team, the decision was overturned on 86 minutes.
A minute into added time, Castillejo looked to steal a draw but Handanovic's reflexes put off the Spaniard, as Danilo D'Ambrosio took Patrick Cutrone's low shot off the goal-line to give Inter the league double.
