Madrid manager Zinedine Zidane made three changes from their goalless draw against Real Sociedad, as Federico Valverde, Casemiro and Luka Jovic were all recalled.
Karim Benzema found the back of the net for Los Blancos in the second minute with a first-time side footed shot into the bottom corner from Ferland Mendy's cross but the goal was disallowed for offside.
Valverde eventually got the opener for the visitors in the 14th minute with a tap-in at the near post after he was picked out by Benzema, who pulled away from the centre of the box to deliver the square pass.
Ramos should have made it 2-0 for Madrid two minutes later after a rebound inside the box fell kindly in his path in the six-yard box and with the net gaping but he blasted a left-footed volley slightly wide of the corner.
After a slow start, Manuel Pellegrini's finally levelled matters ten minutes before the break after Aissa Mandi powered home a header from Sergio Canales' teasing cross at the far post.
Soon after, Betis took the lead with eight minutes left of the first stanza after William Carvalho controlled Nabil Fekir's pass and rifled an effort past Courtois at the near post.
Benzema had a good opportunity to level matters for Zinedine Zidane's troops on the stroke of half-time after turning around the defence to go clear on goal but his low shot was saved by goalkeeper Joel Robles, from close range.
Los Blancos quickly got back into the game just three minutes from the break as a teasing cross by Daniel Carvajal forced Emerson to guide it into the back of his own net in an attempt to clear the ball the away close to Benzema.
Betis were then reduced to ten men in the 67th minute after Emerson fouled Jovic, who was going clear on goal and after the referee reviewed the incident on the Video Assistant Referee monitor, the defender was shown a straight red card.
Things got worse for Pellegrini's men in the last ten minutes as were handed Madrid a penalty for a handball by Marc Bartra following VAR's intervention.
Ramos stepped up to chip a cheeky but confident penalty into the top corner to seal all the three points for the defending Spanish champions.
