The Argentine suffered a knee injury in Barcelona's La Liga match against Las Palmas on the weekend, one that could rule him out for up to eight weeks.
Messi last missed a match against Real in 2007, a 1-0 loss, when the star was just 21 years old.
His injury could see him miss 12 games for the Catalan giants, including the eagerly awaited El Clasico on 8 November.
According to the 28-year-old's brother Rodrigo, Messi wouldn't want to miss that clash, but will only be in action when he's fit enough.
"Leo doesn't know if he'll play against Madrid, but, of course, he'd want to," the sibling told Onda Cero.
"He'll be back when he's ready."
Messi himself took to social media to thank everyone for their well wishes, admitting that being sidelined is difficult.
"Thank you all for your supportive messages," he wrote on Facebook.
"For anyone who loves football, the hardest thing is not to be on the pitch. Now, it's time to recover properly to come back. A big hug."
