A goalless draw in Germany on Thursday night was enough for the Italian club to cement their spot in the final following their 1-0 victory in the first leg in Rome last week.
The outcome means Mourinho will have a chance to win yet another European trophy with Roma, who claimed the UEFA Europa Conference League title last year.
As the full-time whistle blew, the ex-Real Madrid and Manchester United manager was reduced to tears, but said in a post-match press conference this latest achievement is not about his personal record.

"My concern isn't marking my place in the Roma history books," he said, as per Sky Sports.
"It's helping these kids (players) to grow, to achieve important things. It's also helping the Roma fans who have given me so much since day one.
"It's an immense joy to get to another final. The boys give everything, this game is the result of our work, experience, tactical wisdom and knowing how to stay in matches. It's an incredible team."
Mourinho has claimed five pieces of silverware in European competition in his trophy-laden career, including two Europa League and two UEFA Champions League titles.
