Striker Keletso Makgalwa was stretchered off in the 1-1 draw with Cape Town City on Wednesday following late challenge from Thamsanqa Mkhize.
The club have now confirmed that the 22-year-old will undergo surgery on Thursday and is set to miss three to six months of action.
"It has now been confirmed by our medical team that ‘Aguero’ will be ruled out for between three and six months as his road to recovery from his ankle injury begins," the club's statement read.
"Makgalwa has been scheduled for surgery this coming Thursday on his high ankle sprain and will be working at the club’s new high performance centre with a physiotherapist."
The striker, known as Aguero, was on target in Sundowns' MTN8 come-from-behind victory over Bloemfontein Celtic last weekend.
