Hazard joins Los Blancos after seven years with the Premier League side during which he has made 352 appearances and lifted six titles.
The 28-year-old joins Real, as confirmed by the club via their social media channel.
He has signed five-year deal with the Spanish giants, with reports suggesting Real will transfer €100 million (R1.66 billion) plus add-ons for his services to Stamford Bridge.
The winger leaves Chelsea after making 352 appearances for the Premier League side, scoring 110 goals and assisting in another 92. In addition, he was named Chelsea Player of the Season four times.
Hazard also becomes Chelsea most expensive departure, replacing the previous record sale of Diego Costa for €66 million.
His €100 million arrival also makes him Los Blancos' second-most expensive signing in history, with Real splashing more on Hazard than they did in 2009 on Cristiano Ronaldo.
